Do I Need A Referral to Schedule An Appointment?
Yes, a referral is required from your PCP or previous psychiatrist or therapist.
What Should I Bring To My First Appointment?
Bring the following to your first appointment:
- Insurance card(s)
- Picture ID
- Form of Payment
- Complete New Patient Forms (either complete online or printed)
NOTE: You can find our new patient forms under New Clients->Forms where you can either print the forms and fill out by hand or complete them online.
